Information Request Form

If you are ordering a plaque, please send an email to stan@awardsgreen.com including : 1) your name, address, and phone number; 2) the shipping address for the plaque;  3) the invoice address and contact if different than your own; 2) the name of the missionary as it should be shown on the plaque; 2) the mission to which he/she is going, 3) the ward from which the missionary is leaving, 4) the month and year the missionary is leaving, 5) the month and year the missionary will be returning, 6) a scripture reference of reasonable length to be included on the plaque, and 7) any special instructions.
